What these figures are, and are not
Every community here comes from the state licensing register, which publishes licensed capacity — how many residents the license permits — and nothing about how many places are free this week. Ask about openings on the first call.
The published median for assisted living in Nevada is $6,241 a month ($74,895 a year) — 1% above the national median of $6,200 (CareScout (Genworth) Cost of Care Survey, 2025). Where in the state matters more than that average suggests, which is why the figure on each card above is that area's own.

What Medicaid pays in Nevada
Nevada's register does not say which communities accept Medicaid, so ask each one, and ask the state Medicaid office which it contracts with. If the person qualifies, these are the rates the state pays the provider. They are not private-pay prices — what care actually costs here is a different page and a much larger number.

What Nevada Medicaid pays a provider, not a private-pay market price. Nevada publishes a schedule per program rather than one schedule per license, so each license below names the schedule it came from.
Levels and tiers are set by an assessment of the person's needs, not chosen by the family — more help means a higher tier. The communities marked Accepts Medicaid throughout this site are the ones that take these rates; ask any of them directly whether they have a Medicaid opening, because accepting Medicaid and having a bed for it are two different things.

Nevada has no Area Agencies on Aging. That is the phrase on almost every discharge sheet and in almost every article, and in this state it points at nothing: the state's Aging and Disability Services Division “serves as the single area agency on aging”, in its own State Plan for Aging, and its 2016 plan puts it flatly — the designation “means it has no Area Agencies on Aging.” What it publishes instead is the Nevada Care Connection Resource Centers, and between them they cover every county in the state, including the 7 counties where this site lists no licensed home at all. They are free, they are not part of the senior-housing industry, and they handle application help, benefits and eligibility screening, caregiver support and respite. What they do, and who else is paid by whom.

Lyon County
Lyon County appears twice, and that is the state's own list rather than a mistake: the northern center covers it, and the county runs its own line as well. Either is a real answer. Every county list here is printed by the state, with one word of ours in it: the state writes “Carson” and this page writes “Carson City”, which is the jurisdiction's actual name and the spelling Nevada's own licensing register uses. An expansion, and the only thing on this list that is not verbatim.
